pthreads and Windows

It looks like there is a minor error in the #ifdefs in agent++.h and snmp++ 
config.h.

They both contain the following sequence:

#ifndef _THREADS
#define _WIN32THREADS
#define VC_EXTRALEAN
#define _THREADS
#endif

The comments in agent++.h suggest that this should build using pthreads as-is.

However, in order to get it to build properly here, I had to change the 
_WIN32THREADS line to:

#undef _WIN32THREADS

in both the agent++.h and config.h files.
