[SNMP4J] MIB table instrumentation

Hi Eddy,

If you separate the table from the data model, the model implementation
can change (and be simplified) even if the table remains the same.
Because the model is the place to instrument tables for SNMP,
an implementer does not need to implement the table logic
for each instrumentation again.
