[AGENT++] Dynamic MibTable and AgentX++ code [solution]

Hello.

Just in case it helps someone else out there too, the solution of having
dynamic MibTables (ones with the overriden update()) is apparently
simple: you need to add the tables to a MibGroup, and only _then_ add
the group to the SubAgentXMib.

Since apparently this information was not obvious to list contributors,
here it is. Hopefully it will save somebody else a couple of days of
looking at source code and debugging.
